Boys Basketball: Pre-Season Checklist
Tryouts start Monday, February 1, 2022
All of These Items Must Be Completed Prior to February 1, 2022

Current Physical- these are good for two years. If you participated in a sport as a 7th grader, or football/wrestling this year, you are most likely good to go! (Check w/ Mr. Diseth if you are not sure when your physical occurred) If not, you will need to make an appt. with your doctor.

Concussion Screening- these are good for two years. If you played football, wrestling or basketball last year, or football/wrestling this year, you are good to go! If not, you can make an appt. with Olympic Sports and Spine (psdconcussion.osstherapy.com), Mr. Diseth has the information on this.

Electronic Participation Forms- If you already played a sport this year, you are good to go! If not, a parent will need to access this website: puyallupschools.rankonesport.com, and complete the “Athletic Eligibility” and “Parent Permission and Consent” Forms.

Gray Travel Card- If you participated or tried out for a sport this year, you are good to go! If not, pick one up from the office, or from Mr. Diseth and have a parent fill out.

Final Clearance- One you have completed steps #1-4, see Mr. Diseth on the stage during your lunch time in January 25th, 26th and 27th. Bring all of your paperwork (unless it has already been turned in.) Make sure that you are passing all of your classes, and start working out/exercising if you haven’t been.
